About the Organisation
Passport to Health is the local exercise referral service for people who are inactive and have (or are at risk of developing) other long term medical conditions.
The service is for adults aged 16 years and over who are registered with a GP in Bath and North East Somerset.
The programme supports individuals with low activity levels and other long term medical conditions, such as obesity or being overweight, high blood pressure, Type 2 Diabetes, or depression.
All of our physical activity programmes include pre and post-exercise assessments to ensure each individual receives a tailor-made, safe and appropriate activity programme. All assessments are free and are provided by trained and qualified exercise referral specialists.
Your Passport to Health programme can offer a range of physical activity options including 12 weeks of subsidised access to your local leisure centre for gym, swimming and/or fitness classes, weight loss support groups.

How do people access your service?
There will be a small charge for some activity sessions depending on your choice of activity.
If you would like to bring a carer or friend along with you, this will be free of charge and is encouraged.

Who can use your service?
You can be referred to this service by your GP or other health care professional, or you can self refer.
If you do want to self refer, you can do so by calling or emailing the Healthy Lifestyle Service Hub. Please note that to take part in the scheme, we will need to contact your GP.
Once you have been referred, the exercise referral coordinator will contact you to give you more information about the programme and to complete an initial assessment.
