About the Service

About the Organisation

Our Improving Access to COPD Therapies (IMPACT) Team provides services to people who have a confirmed diagnosis of COPD (Chronic Obstructive Pulmonary Disease).

We offer:

•A ‘hospital at home’ service for people with chronic bronchitis and/or emphysema, also known as Chronic Obstructive Pulmonary Disease (COPD)
•An oxygen assessment service for people using the service currently using oxygen who are registered with a B&NES GP and are over 18 years old
•An Exercise and Education programme with classes offering light exercise and education – pulmonary rehabilitation

14-25 Years
Who can use your service?

Any adult over 18 years of age who lives in the Bath & North East Somerset (B&NES) area or is registered with a B&NES GP.

You can be referred to this service by your:
•Practice Nurse
•Community Matron
•Respiratory Consultant
•IMPACT team member
