About the Organisation

Our Diabetes Education sessions provide sound advice, information and encourage people who are at risk of being diagnosed with, or have Diabetes type 2 to be proactive and to take responsibility for day to day management of their condition.

We support adults over 18 years of age who live in the Bath & North East Somerset (B&NES) area or who are registered with a B&NES GP.

Our Type 2 Diabetes Education sessions are available as part of our ‘Living Well’ Programme. There are two separate programmes the first is for people who are ‘At-Risk’ of being diagnosed with Diabetes Type 2 and the second, provides comprehensive advice and support which equips people who use the service with knowledge to enable them to self-manage their condition proactively:

‘Living Well with Diabetes Type 2 (formerly Conversation Map)
This group session provides a wealth of information in a short day programme (9.30 am – 2.45 pm) for those who have Type 2 Diabetes. Sessions are run once a month in venues across the Bath and North East Somerset Area.

Adults over 18 years of age who live in the Bath & North East Somerset (B&NES) area or who are registered with a B&NES GP.

You can be referred to this service by:
•Health Professional (GP, practice nurse, dietician, podiatrist, diabetes specialist nurse)
•Self referral (for Walking Away & Conversation Map Training courses only)
