About the Organisation
This service is for any adult of 18 years and over who has a learning disability, meets the Fair Access to Care Criteria (FACS) and has been assessed and confirmed as eligible for a service from our team.
We can also provide advice and information for young people (16 years and over) and their parents/carers. Please note that the young person would have to have been diagnosed with a learning disability and been referred to us through the Transitions team.
We can help you with:
•Completing an assessment of a person’s health and social care needs
•Setting up a personal budget and supporting the person to arrange a service to meet their needs
•Reviewing a person’s current services and changing them if necessary
•Completing an assessment of a carers needs, including respite care
•Providing advice on benefits, housing, employment, education and training

Who can use your service?
Any adult of 18 years in Bath & North East Somerset and over who has a learning disability and has been assessed as eligible for a service from our team and meets the Fair Access to Care criteria (FACS).
Anyone can refer a person with a learning disability who lives in Bath and North East Somerset and has a GP registered in that area.
A referral will be accepted from a GP or other health or social care professional, or a carer or relative can contact us by letter, telephone or email.
